Print out the application Supply all of the necessary information Return the application to Universal Purchasing Association with your check by mail. Or, if you want to get started today fax (864-879-2937) the application with your credit card information. Note: If you choose to process the membership fee by credit card the transaction will not be processed until the membership application has been approved.
